Enter your User ID in the box that says username.

• The Opportunity Passport Coordinator will provide you with your User ID when you enroll.

Enter your password and hit submit.

• The Opportunity Passport Coordinator can re-set your password if you have forgotten it.

Click “Fill Out New Survey.”

Complete the survey and hit “Submit.”
